Have you ever felt a sharp, electric pain shooting from your lower back down through your buttock and into your leg? That means you have probably experienced "sciatica"
Sciatica is one of the most misunderstood conditions in musculoskeletal health. So let's clear the air. Here at Urban Physiotherapy we are here to genuinely help, and give you the right physio approach sciatica can get you moving again.
What is sciatica?
Sciatica describes pain that travels along the path of the sciatic nerve, the largest nerve in your body. It begins in your lower spine, runsning deep through the buttock, and continuing down the back of the leg towards the foot.
When something irritates or compresses this nerve, you feel the classic radiating leg pain, sometimes accompanied by pins and needles, numbness, or weakness. The actual cause of that irritation varies. It might be:
A bulging or herniated disc pressing on a nerve root
Narrowing of the spinal canal (stenosis)
Tightness in the surrounding muscles
Age-related changes in the spine.
Identifying your underlying cause matters, because it shapes the best path to recovery.
Myths that hold people back
Myth 1: "A disc has slipped out of place."
Discs don't slip. They're firmly anchored between your spinal vertebrae. What actually occurs is that the soft inner material of a disc bulges or herniates, sometimes contacting a nearby nerve. With the correct physiotherapy management, disc herniations frequently shrink and reabsorb over time. Many people with herniations on a scan may not have pain at all.
Myth 2: "Pain means damage."
Pain is not always damage, most of the time it is your nervous system's alarm. When sciatic nerve pain lingers, it's often because the nerve has become irritated and sensitised, not because tissue or structures are being torn. For people experiencing pain, hurt does not necessarily equal harm, and gentle, progressive movement is safe even when it feels uncomfortable.
Myth 3: "Rest is the best medicine."
For decades, bed rest was the standard advice. We now know it's one of the worst things you can do. Prolonged rest stiffens joints, weakens muscles, and tends to make the nervous system more sensitive, not less. Current evidence is clear that staying active, within sensible limits, speeds recovery.
Myth 4: "Surgery is inevitable."
The vast majority of sciatica cases settle with conservative care, often within six to twelve weeks. Surgery is reserved for a small minority, those with severe, persistent symptoms or significant nerve compression. For most people, the path forward is movement, education, and a well-structured rehab plan.
When sciatica is not something to wait out
While most sciatica is benign and improves with time, a small number of symptoms are genuine red flags that need urgent medical attention if you experience any of the following:
Loss of control over your bladder or bowels
Numbness around the inner thighs, buttocks, or genital area (the "saddle" region)
Progressive or significant weakness in one or both legs
Numbness or weakness affecting both legs at once
These can signal a rare but serious condition that requires prompt assessment.
How Urban Physiotherapy Can Help
Sciatica can be frightening, but it is rarely as dangerous as the internet makes it sound. For most people it is a temporary, treatable condition that responds beautifully to staying active, the right exercises, good information, and a clear plan. The biggest obstacle to recovery usually isn't the nerve itself, it's the fear and misinformation surrounding it.
You don't have to figure this out alone.
At Urban Physiotherapy, we don't believe in one-size-fits-all sheets of generic stretches or vague reassurance. We believe in understanding your pain, its specific cause, its triggers, and what your body needs to recover with confidence.
Here's what working with our team looks like:
A thorough assessment. We take the time to listen to your story and carry out a detailed physical examination to pinpoint what's actually driving your sciatic nerve pain
