ACL - Prehab

What you do before your ACL reconstruction can make a big difference to how fast and how well you recover. Our ACL prehab program helps you build strength, improve flexibility, and reduce swelling before surgery — setting you up for a smoother recovery afterwards.

Goals:

  • Regain full knee motion and control

  • Reduce swelling and stiffness

  • Strengthen quadriceps, hamstrings, and glutes

  • Learn post-surgery exercises and expectations

Treatment Includes:

  • Targeted strength and mobility training

  • Hands-on physiotherapy to optimise joint movement

  • Swelling and pain management

  • Gait retraining and education on recovery phases

ACL Four Phase Rehab Program 

Phase 1:

Early Recovery

 

Weeks 0 - 4 

8 x  Physiotherapy Sessions (Twice Per Week)

 

Objectives:

  • Minimise pain and inflammation

  • Achieve full knee extension and at least 120° flexion

  • Regain quadriceps activation and muscle tone

  • Normalise walking pattern (gait)

  • Prevent scar tissue formation and stiffness

Treatment Approach:

  • Hands-on physiotherapy to manage swelling and soft-tissue tension

  • Cryotherapy and compression for inflammation control

  • Electrical muscle stimulation (if needed) for quadriceps activation

  • Assisted range-of-motion exercises

  • Gait retraining and crutch weaning

  • Patella mobility and early strengthening (straight-leg raises, heel slides)

Phase 2:

Strength & Mobility 

Weeks 4 - 8 

6 x Physiotherapy Sessions (1-2 Per Week)

Objectives:

  • Regain full range of motion (flexion and extension)

  • Build early muscle strength and endurance

  • Re-establish neuromuscular control and single-leg balance

  • Improve confidence and stability during everyday movements

Treatment Approach:

  • Progressive resistance training (leg press, hamstring curls, step-ups, squats)

  • Stationary bike and closed-chain kinetic exercises

  • Balance and proprioception work (wobble board, single-leg stance)

  • Manual therapy for joint and soft-tissue mobility

  • Core strengthening and hip stability training

  • Education on safe load progression

Phase 3 

Functional Strength 

Weeks 8- 24

12 x Physiotherapy Sessions (over 16 weeks)

Objectives:

  • Achieve near-symmetrical lower limb strength

  • Restore dynamic stability and control in single-leg movements

  • Begin straight-line running and light agility work (when cleared)

  • Build capacity for higher-impact loading

Treatment Approach:

  • Progressive gym-based strength program (lunges, split squats, Romanian deadlifts, lateral movements)

  • Advanced balance and control drills (Bosu, single-leg reach, perturbations)

  • Early plyometric work (mini hops, skipping, landing mechanics)

  • Cardiovascular conditioning (bike, elliptical, treadmill intervals)

  • Running re-introduction (typically around Weeks 16–20 once objective criteria are met)

  • Functional performance testing to monitor progress

Phase 4

Return to Sport

 

Weeks 24 - 40 

8 x Physiotherapy Sessions (Once a Fortnight)

Objectives:

  • Achieve full power, endurance, and agility

  • Prepare for return to cutting, pivoting, and jumping sports

  • Complete formal return-to-sport testing battery

  • Build psychological readiness and confidence

Treatment Approach:

  • High-intensity strength and power training (weighted squats, deadlifts, sled pushes)

  • Plyometric progression (box jumps, single-leg hops, bounding drills)

  • Sprint, acceleration, and deceleration drills

  • Agility and change-of-direction training (ladder, cone, and sport-specific movement)

  • On-field or court-based rehab (as appropriate to sport)

  • Return-to-sport testing, including hop tests, movement quality analysis, and strength benchmarks
